What a PSA Result Means: Numbers, Cutoffs and Causes

Quick answer

PSA is a protein made by the prostate, and a blood test measures it. A level over 4.0 ng/mL is generally treated as abnormal by the NCI, with age-based cutoffs used by some doctors. BPH and prostatitis can raise it, finasteride and dutasteride lower it, and a raised result calls for more questions, not a conclusion.

The basics01

What is PSA?

Prostate-specific antigen is a protein made by the cells of the prostate, normal and abnormal. A small amount leaks into the blood, where a test can measure it in nanograms per millilitre, written ng/mL.

The test is simple, a blood draw, and can be done at any clinic. What the number means is not simple. PSA is specific to the prostate and not specific to cancer, which is the heart of every debate about it.

The National Cancer Institute explains the cutoffs and the many reasons the level can change (NCI, PSA test).

The evidence02

What can move the number

Factors that raise or lower a PSA result
FactorEffect on PSA
Enlarged prostate (BPH)Can raise (NCI, PSA test)
Prostatitis or urinary infectionCan raise (NCI, PSA test)
Finasteride, dutasterideLower; doctors adjust the cutoff (NCI, PSA test)
AgeTends to rise with age
Recent ejaculation or cyclingMay raise briefly
Prostate cancerMay raise, or not at all

The numbers03

What is the usual cutoff?

For the cutoff, the NCI points to 4.0 ng/mL as the general mark of an abnormal result, while some doctors apply lower, age-based figures (NCI, PSA test). A level below it is not a guarantee of health, and a level above it is not a diagnosis of cancer. Many men with a raised level turn out to have a benign cause.

The trend matters too. A level that rises steadily over several tests is more telling than a single high reading, and doctors often repeat a test before acting.

The catch04

Should everyone have the test?

The NCI says PSA is not recommended for routine screening of the general population, and men who think about it should discuss risks and benefits with a doctor (NCI, PSA test). The USPSTF makes the decision individual for men 55 to 69 and advises against it from 70 (USPSTF, Prostate cancer screening).

The reason is that screening can find slow-growing cancers that would never have caused harm, leading to biopsies and treatments with side effects. Our checkups guide sets out the age bands.

Safety05

How to get a cleaner result

  • Tell the lab about finasteride, dutasteride and any supplement.
  • Avoid ejaculation and long cycling for a day or two before the test.
  • Report fever, urinary infection or a recent procedure, since these can raise PSA.
  • Ask whether to repeat a high result after a few weeks before further tests.

What to do06

What happens after a high result?

Usually a conversation, a repeat test and sometimes a urine test or a scan, such as an MRI of the prostate, before any biopsy. Many results settle without further action. Others lead to a biopsy, which has its own risks, and doctors weigh them with you.

Do not try to lower the number with a supplement before the test. A change in the number without a change in the cause is a risk, not a cure, and it can hide a problem.

What about supplements and PSA?

Even at high doses, saw palmetto left PSA no different from placebo in the sources we cite (MSK, Saw Palmetto). Pomegranate data are conflicting (MSK, Pomegranate). Still, tell the person who orders the test about everything you take. See our post on Prostadine and PSA tests for more.

In practice08

How age changes the picture

PSA tends to creep up as the prostate grows with age, which is why some doctors use age-based ranges and not a single cutoff. A level that would be unremarkable at 75 may deserve a closer look at 50, and the same reading means different things for different men.

Because of this, ask the doctor how your number compares with others of your age, and what the trend has been. A baseline taken when you are well gives later results something to be compared with, which is worth more than any single reading in isolation.

The basics09

What to do while you wait for results

Waiting is the hard part, and it is common to search the internet for numbers and feel worse. Try to resist comparing your result with forum posts, since the same figure can mean very different things in different men and settings.

The evidence10

Why one number should not drive a purchase

A raised or a low number is sometimes used in marketing to sell a product, with the suggestion that a supplement can bring it down or keep it healthy. No source we cite shows that a supplement lowers PSA in a way that improves health, and a lowered number can hide a cause that needs attention.

Treat any such claim with caution and read our guide to checkups and tests first.

FAQ11

Prostadine questions people ask

Is there a normal PSA number?

There is no single normal. The NCI treats readings over 4.0 ng/mL as generally abnormal, some doctors use age-based cutoffs, and many benign factors raise the number.

Can a high PSA be caused by an infection?

Yes. Prostatitis and urinary infections can raise it. Doctors often repeat the test after treatment before drawing a conclusion.

Do I need a PSA test?

It is an individual decision after talking to a doctor, especially for men 55 to 69. The USPSTF advises against routine screening from age 70.

Does finasteride affect PSA?

Yes, it lowers it, so doctors use a lower cutoff for men taking it. Always tell the lab and your doctor that you take it.
